Acute inpatient rehabilitation

Rehabilitation desk at Blake Medical Center

Our inpatient rehabilitation unit offers 28 private and semi-private patient rooms and is dedicated to providing comprehensive rehabilitative care to patients in Bradenton. As part of an inpatient program, patients always have access to a higher level of care, if needed, such as dialysis, diagnostic imaging, medical consultations or laboratory testing.

In fact, if a medical emergency occurs while a patient is anywhere in the inpatient rehabilitation unit, a Rapid Response, or Code Blue, may be called to activate a highly specialized medical team to the patient's side. The team assesses the situation, calls in necessary specialists, completes any diagnostic testing and makes further medical recommendations.

Discharge planning

Discharge planning is an important step in the rehabilitative process. Upon discharge from our center, equipment and home or outpatient services are coordinated to complete the final stages of the rehabilitation program. Your rehabilitation team is led by a social worker who helps plan and track your recovery from the day of admission to the day you are discharged.

Outpatient rehabilitation services

Outpatient therapies are offered at Blake Outpatient Therapy located on our hospital's main campus in Blake Park. Our outpatient therapists hold various certifications and possess specialized training to help patients achieve their rehabilitation and recovery goals. Similar to our inpatient services, we offer multiple therapies for our outpatient services, including:

Support from and for family and friends

As patients adapt to change, the support of family and friends is vital, but they face their own challenges with the sudden onset of change for their loved one. We help by including support for the patient's family, caregivers and friends, in our individualized therapy care plans. All members have an opportunity to learn about their role and each is asked to participate in therapy training sessions as needed.
