Become a Healthy Choices Participating Restaurant!
Join us as we work together to make our community healthier. Fill out the form below for information on becoming a Healthy Choices participating restaurant. Participation subject to approval. Restaurant must be located within Manatee County. Menu items are created in partnership with restaurant and Registered Dieticians at Blake Medical Center and are considered healthy for the average adult. There is no cost to participate in the program, however restaurants may incur some incidental costs.
